Clarify licenses

- add license headers to everything
- convert a few files from public domain or LGPL2.1+ to GPL2+
- some archaeology to clarify which files are from which library

A lot of header clean-up.

These are roughly equivalent to autoconf's AC_HEADER_STDC and even they
say that macro is obsolescent.

strings.h is not really a C89 header, but it defined some things that
are no longer needed [1]. And no file actually #includes it.
